Luxottica Warranty and Repair Service Center
As a courtesy to our valued Ray-Ban USA customers, we provide repair service for any product not covered or outside our warranty period (as indicated in the Warranty claims page).
1. To start a claim, download and complete this Repair Request Form.
2. Mail your eyewear and completed Repair Request Form to “Luxottica Warranty and Repair Service Center” for full and prompt evaluation. Please note that if you are requesting an out of warranty repair it is not necessary to enclose proof of purchase.

Please include the following items:
• Your frame in a protected shipping carton
• The Repair Request Form, COMPLETELY FILLED IN with ALL your details
3. You will receive a notification email/letter advising of receipt of eyewear, your notification number and claim status.
4. Please allow 10-14 business days for our trained technicians to identify the cause and degree of damage. After the evaluation, you will receive notification via email or letter outlining the outcome of the evaluation and the charges to cover the cost of spare parts. A $15.00 processing fee to cover handling and evaluation will also apply. You will be asked to contact our call center to communicate your acceptance of the offer and if so to proceed with the payment.
5. Please allow up to 4 weeks for evaluation, repair and return shipping of your repaired or replaced eyewear.

● We DO NOT warranty or service prescription lenses. Therefore, we ask that you please remove prescription lenses before sending your eyewear. In the case that you are unable to remove your prescription lenses, please download, sign and send this Rx Waiver Form along with your eyewear. We are not responsible for any prescription lenses that are sent with frames.
● Protection is important, so please place the eyewear in a case, and the case in a cardboard box, in order to protect the product during transportation. We recommend sending your eyewear as certified or insured for tracking purposes. Please note we ARE NOT responsible for any lost packages, for any damage in transit or damages due to improper packaging.
● Please DO NOT include any form of payment (cash, check or money orders) along with your eyewear.
● Failure to respond within 30 days from the notification receipt will void all offers.
● Original eyewear will be returned at your cost only upon request within 90 days. Failure to request return of your eyewear within 90 days will authorize Luxottica to dispose of your eyewear. Luxottica assumes no liability.
● Please note that any replacement is considered final sale.
● All repairs/replacements are guaranteed for 90 days.
